Links2Health is a comprehensive program designed to provide wraparound care to community members to help them meet essential life needs and improve health outcomes. Connect individuals to Community Supports to meet their social needs, including housing navigation, housing retention and sustaining services. Support connecting to housing, linking up with a primary care home, accessing transportation to appointments or a host of other social services.

Eligibility
Clients must be age 18+, live in Santa Cruz County, have active coverage with Central California Alliance for Health (CCAH), and have Medi-Cal in order to be considered for the program. Eligible populations include: Adults or families experiencing homelessness Adults with Serious Mental Illness (SMI) or Substance Use Disorder (SUD) Adult high utilizers of the healthcare system (multiple hospitalizations) Adults at risk for long-term care at a nursing facility or other rehabilitation center Adult Nursing Facility residents transitioning to the community Adults with an intellectual/Developmental disability Pregnant and Post Partum individuals including Birth Equity Population of Focus Adults transitioning from incarceration
