Transitional Housing Plus is a supportive housing program that serves former foster and probation youth between the ages of 18-24. Participants receive assistance securing independent housing in the community and meet regularly with their THP Coordinators to work on independent living goals. Throughout this process, participants receive financial assistance with rent, utilities, food, and educational expenses while also saving money for when they leave the program. They also receive emotional support, life skills coaching, and connections to valuable community resources.

Provides crisis intervention, advocacy, case management, and protective shelter for crime victims and survivors. Offers comprehensive legal and immigration assistance, including restraining orders, court accompaniment, and U-Visa support. Administers support groups and restorative justice practices to facilitate healing and system navigation.

6-month Shelter for single women and women with children (males under the age of 12). 1 to 2-year Transitional shared Housing that can accommodate males over the age of 12, including fathers.

Offer a rotating nightly shelter program hosted at various faith communities in Santa Cruz County.

Provides Women's sober co-Living, transitional housing

Identifies and connects unaccompanied youth and young adults ages 15-24 who are unhoused or at risk of homelessness to individualized community services. Provides case management, tailored support planning to achieve housing, education, and employment goals.

Provides an online resource directory to assist individuals with locating current emergency shelter and Sober Living Environment (SLE) bed availability. Offers directory listings for open waitlists for low-cost housing units.

Provides mobile showers available to anyone in need.

The Family & Children's Services Transition Age Youth (TAY) program serves current and former foster youth age 15-24 with a range of services to assist youth in making a healthy transition to adulthood. Programs include counseling, skill-building, a drop-in center and subsidized supportive housing.

TAY programs include: • The Independent Living Program (ILP) serves current and former foster and probation placement youth age 15-21. It assists youth to develop independent living skills to successfully transition to self-sufficiency and includes services such as one-on-one counseling, assistance in obtaining ID cards, driving permits and licenses and workshops on topics that assist youth in achieving personal, education and vocational goals. • The Transitional Housing Program is a supportive housing program for former non-minor dependents and foster youth between the ages 18-24. Youth are supported with financial assistance, educational expenses, emotional support, life skills coaching, and referral to community resources to help them achieve the financial stability necessary to succeed in independent living. The UCSC Slug Support Program was created to promote early intervention with students of concern to prevent culminating problems from escalating into a crisis. The goal of the Slug Support Program is to identify students navigating difficult or challenging situations and assist them in resolving their concerns. Slug Support also has emergency food and housing resources for students as well as a financial assistance hardship fund for basic needs. Slug Support serves both undergraduate and graduate UCSC students.

The Outreach-Based Case Management Program helps those who are experiencing homelessness in Santa Cruz County, pursue permanent housing solutions, access to community health and financial resources, provide housing navigation and supportive services to help individuals and families lead more stable lives. The Outreach-Based Case Management team consists of an Program Manager, Case Managers, and Healthcare Navigators. Healthcare Navigators focus on

• breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities,
• introducing a better way to coordinate health care, and
• providing high-need members with in-person care management where they live.
