Description: Cleveland House is a beautiful 6-bed, female-only facility which provides a safe environment for likeminded sisters in recovery. Located near busy Mission Street in Santa Cruz's beautiful Residential West Side, residents will have access to many local eateries, businesses, activities and job opportunities. Residents are encouraged to live independently in this supportive, community based setting. Residents are encouraged to develop and practice life skills and relationship building. FREEDOM HOUSEDescription: Located just off historic Highway 1 in Aptos, CA, Freedom House is a sober living home for 12 men who are actively engaged in recovery. The multi-story home sits on a large wooded lot, in a quiet and serene setting. Freedom House is located far away from the hustle and bustle of Santa Cruz, with convenient public transportation access. At Freedom house, clients receive the benefits of community support from recovery peers as well as bilingual house management. Residents build community connections by attending weekly house meetings where rules and policies are frequently reviewed and assessed which helps to ensure a safe and supportive environment for all residents. Our certified clinicians provide a full spectrum of trauma-informed substance use disorder services for youth and their families. As an integrated behavioral health agency, we also specialize in treating co-occurring disorders. Harm reduction is central to our approach, and we use a number of evidence-based modalities. Mindfulness and body awareness activities are also integrated into our programs.

Group Counseling: Evidence-based group models, such as Seven Challenges and Seeking Safety, are offered at our north and south office locations.

Casa Pacific is part of an innovative project centered around work and meaningful daily activities, as the core of the co-occurring disorders treatment approach. Residents attend the Community Connection's Mariposa Center to learn vocational skills and participate in various volunteer jobs, or they participate in the Community Action Board's Alcanse work crew. Other activities include socialization, recreation, accessing community resources, on-site recovery meetings and individualized attention to assist residents in maintaining sobriety, reaching their personal goals, and transitioning into independent community living

Provides comprehensive, strengths-based, culturally and linguistically appropriate behavioral health services for Medi-Cal eligible children and youth who have moderate to severe behavioral health needs.

Services include assessment, treatment planning, psychotherapy (individual, family), case management, and psychiatric care (evaluation and medication management). Services delivered at County behavioral health clinic locations, community, or home-based settings, as clinically appropriate.

The Access Team provides 24-hour confidential crisis intervention and referral services (800-952-2335).

Provides substance use disorder prevention, early intervention, and treatment services across the continuum of care.
