Families Together provides counseling, parenting support, and case management to families using a home visiting model and a client-centered approach to services. Caregivers participate in the development of an individualized service plan based on their unique strengths and needs.

Families referred by Santa Cruz County Family & Children's Services or CalWORKs are assigned a therapist who helps design customized services, including sessions in English or Spanish, and in the home or other preferred location. Families learn about parenting, child development, parent-child attachment, and general wellness. In addition to counseling, home visitors connect families with other community resources such as substance use counseling, interpersonal violence services, and early education support.

The Encompass Housing Navigation Team provides support to tenants of Encompass Housing in order to facilitate a stable living environment, help assist tenants in developing a support system, improve skills needed to maintain housing, and to connect with community resources for employment, education, medical needs and community involvement.

FUERTE Wraparound is a four to twelve month intensive program that aims to address the mental health needs of youth on probation (ages 12-20) and increase parental capacity and skills to care for and address the mental health needs of their children involved in the justice system. Additionally, FUERTE Wraparound addresses the unmet needs of family members through linkages to community resources.

Services include individual and family counseling in the home, and intensive case management in the home and community settings, and 24-hour crisis response. Also included are linkages/referrals to mental health advocacy groups (NAMI-National Alliance on Mental Illness, MHCAN), medication management, therapeutic groups, parent education and support, peer support, pro-social connections and outpatient substance use treatment.

Provides a variety of diagnostic and prevention services to help those affected by severe autism, mental retardation, cerebral palsy, uncontrolled seizures and other developmental disabilities. Services include;
  • Case management and service coordination including planning, goal setting, residential placement, respite help, medical services, therapies, parenting classes, independent living skills training, mobility training and more.
  • Early intervention services for young children (age 0-3) with developmental disabilities may include speech therapy, occupational therapy, physical therapy, infant development programs, behavior modification and respite care.
  • School age services are provided for children and youth (age 3- 22). Offers assistance accessing generic resources, advocacy at Individual Education Plan (IEP) meetings, respite and camp, day care, residential placement, behavior modification and adaptive skills training.

Adults' services revolve around community inclusion, self-determination, and independence. Services deemed appropriate to meet an individual's needs may include
  • Assistance accessing community resources or entitlements such as SSI, day programs, supported employment, independent living skills training, supported living services, residential care home placement, respite, behavior modification, adaptive skills training and competency training (for court-related issues).
