Description: Located just outside of Downtown Santa Cruz, CA, this 8-bed, all male facility provides a healthy balance of independence and support. The location provides easy access to public transportation and is in the heart of West Side Santa Cruz, which is bustling with activities and local businesses. CLEVELAND HOUSE
Description: Cleveland House is a beautiful 6-bed, female-only facility which provides a safe environment for likeminded sisters in recovery. Located near busy Mission Street in Santa Cruz's beautiful Residential West Side, residents will have access to many local eateries, businesses, activities and job opportunities. Residents are encouraged to live independently in this supportive, community based setting. Residents are encouraged to develop and practice life skills and relationship building. In this supported environment, residents are able to develop a stronger sense of accountability, responsibility and community to further their recovery. FREEDOM HOUSEDescription: Located just off historic Highway 1 in Aptos, CA, Freedom House is a sober living home for 12 men who are actively engaged in recovery. The multi-story home sits on a large wooded lot, in a quiet and serene setting. Freedom House is located far away from the hustle and bustle of Santa Cruz, with convenient public transportation access. At Freedom house, clients receive the benefits of community support from recovery peers as well as bilingual house management. Residents build community connections by attending weekly house meetings where rules and policies are frequently reviewed and assessed which helps to ensure a safe and supportive environment for all residents. S Se Puede is one of the only bilingual and monolingual treatment environments in the nation. This 23-bed residential program serves Spanish speaking, male clientele for a 30-90-day period, based on individual client needs. Staffed by a bilingual team, highly skilled, trained, and certified clinicians help clients focusing on issues surrounding recovery from substance use and co-occurring disorders. Program activities include evidence-based trauma informed and cognitive behavioral therapies (CBT), Dialectical Behavioral Therapy (DBT), as well as focused individual and group counseling, pro-social life skill building, mindfulness, and daily house meetings.

Provides comprehensive, strengths-based, culturally and linguistically appropriate behavioral health services for Medi-Cal eligible children and youth who have moderate to severe behavioral health needs.

Services include assessment, treatment planning, psychotherapy (individual, family), case management, and psychiatric care (evaluation and medication management). Services delivered at County behavioral health clinic locations, community, or home-based settings, as clinically appropriate.

The Access Team provides 24-hour confidential crisis intervention and referral services (800-952-2335).

Provides substance use disorder prevention, early intervention, and treatment services across the continuum of care.
