TRANSITIONAL AGE YOUTH HOUSING NAVIGATION PROGRAM

Provides housing navigation and housing focused case management to young adults ages 18-24 who were formally in the Foster Care System. We provide one on one case management, housing navigation, resource connection, provide life skill training to obtain and maintain housing, along with mentorship and individualized planning.

Application process

Eligible Clearance through FCS Schedule Intake with Case Manager Submit FUP Voucher Application Once Voucher is approved, then focus on the direct housing navigation services. Continue After care case management to promote sustained housing.

Eligibility

18-24 Exited Foster Care or Probation Services after the age of 16. Must be either At Imminent Risk of Homelessness or facing literal Homelessness.
