Eligibility

Must be a California resident, Medi-Cal eligible, submit a completed Health Care Certification (SOC 873) form and not be a resident of a facility. For errand running shopping assistance/Personal care: Medi-Cal recipients 65+ and/or disabled with licensed health care practitioner certifying the need for in-home care. Must be: -Medi-Cal eligible -If not already receiving Medi-Cal, individual will need to apply for it.
  • Must also live in your own home or a dwelling of your choice.
-Blind, disabled or 65 years of age or older living in a home, apartment or residence of choice (not including a hospital, nursing home, assisted living or licensed care facility) and unable to live safely at home without care

Application process

Begin the process by completing an IHSS application (form SOC 295). A completed Health Care Certification (form SOC 873) signed by a licensed health care provider will also be required.
