Eligibility
Must be a Medi-Cal member.
Qualifying life circumstances include:
-People experiencing or at risk of homelessness
-Frequent hospital or emergency department users
-Individuals with serious mental health or substance use challenges
-People with complex medical conditions
-Individuals who are pregnant or postpartum
-People at risk of institutionalization
-Children and youth involved in foster care or child welfare services
