Provides case management and social support to persons living with memory loss. Participants engage in social, cognitive and physical activities. Works to enhance quality of life and create connection through community. Program also allows for caregivers to have a break from their caregiving duties. Nutritious lunch provided to all on-site participants. Services include:
- Coaching - on-demand dementia companion and coach and home safety checks
- Case Management - personalized care plans
- Home Care - extra help at home and aides build a relationship with loved ones
- Enrichment Center - center provides physical and mental stimulation
