Serves the Pajaro Valley community with a focus on the Pajaro Valley Unified School District(PVUSD) youth and their families offering counseling, substance abuse (drug, alcohol, and tobacco)prevention and intervention, and conflict resolution. Mental health services, substance use disorder services, prevention services, youth leadership development, family-supporting services. Collaborates with police department, public probation and education department, a community-based gang prevention program (BASTA) to provide prevention curriculum, student gang intervention services, school dropout, teacher in service and parent education in PVUSD schools and community.

Provides case management, systems navigation, substance use resources, mental health services, and mutual support groups for crime victims and survivors. Supplies basic needs assistance, family services, and food and diaper distributions. Administers educational interventions, attendance plan implementations, and special education advocacy support.

CAFES was launched to close a gap in diversion opportunities for Santa Cruz County adults who are first-time offenders with little or no criminal history and who need mental health, substance use and other related services that had been inaccessible prior to Prop 47.

Provides 43 low-income housing units with various layouts for individuals and families. Offers efficiency, 1-bedroom, 2-bedroom, and 3-bedroom or larger layouts.

Coordinated Entry is a systemic approach to connecting people experiencing homelessness with available assistance in the community. It is an important way to ensure fair and equitable access to limited housing resources and is a Federal and State requirement. The goals of the new Coordinated Entry System are to:

(1) Facilitate connections to mainstream and community services for as many persons experiencing homelessness as local resources allow;

(2) Streamline the process for matching to limited housing resources within the Housing for Health Partnership network (CoC); and

(3) Prioritize resources for households with the most significant barriers to getting and keeping housing and for those with the greatest personal health and safety risks.

H4HP Connectors may work as part of an outreach team, drop-in center, multi-service center, or other program serving people experiencing or at risk of homelessness. They engage with persons experiencing homelessness, build rapport, enroll participants in HMIS programs and collect participant data.

Connectors offer problem solving assistance with all participants and may conduct a Housing Needs Assessment (HNA). The HNA provides Connectors and participants with information needed to create and act together on an individualized Housing Action Plan and provides information to determine which participants are eligible and prioritized for H4H-supported housing and services.

Only the highest priority households are added to the queue for the system's limited housing resources.

Provides a rotating nightly shelter program hosted at various faith communities in Santa Cruz County.

Provides organized soccer activities and athletic training for at-risk youth. Administers peer and adult mentorship, life skills training, and educational support services to promote personal growth and career readiness.

Provides comprehensive legal and immigration assistance for crime victims and survivors. Offers specialized support with preparing victim impact statements and securing U-Visas.

Matches youth facing adversity with adult role models, in a positive mentoring friendship. Each adult volunteer mentor gives their youth partner, their time, friendship, and understanding. Professional caseworker support is ongoing to monitor the growth of each individual match.

Matches qualified care providers with recipients of the IHSS program who need assistance with light housekeeping, laundry, meal preparation, clean-up, shopping, and errands as authorized.

Provides intensive early intervention services for children with learning delays. Offers parent training and education covering positive parenting practices and behavioral strategies to support children with autism and developmental delays.

Provides screening and enrollment assistance to community members seeking Medi-Cal coverage. Assists participants with understanding eligibility requirements, completing applications, and updating case information. Supports current Medi-Cal enrollees with renewal and understanding recent and future changes to Medi-Cal eligibility.

Provides public benefits enrollment assistance for CalFresh, CalWORKS/TANF, WIC, and State Unemployment Insurance.

Operates food distributions of produce and non-perishable food items at 100 locations countywide.

Supplies food to 100 local nonprofits so they don't have to source food themselves Also provides bilingual nutrition education, Cal AIM healthy food boxes, CalFresh outreach, and application assistance. Supports clients during Public Safety Power Shutoff (PSPS) events

Provides nurturing and affordable early education services for children during the first 5 years of life. Administers services across 6 centers to support child health and development.
