      The Outreach-Based Case Management Program helps those who are experiencing homelessness in Santa Cruz County, pursue permanent housing solutions, access to community health and financial resources, provide housing navigation and supportive services to help individuals and families lead more stable lives. The Outreach-Based Case Management team consists of an Program Manager, Case Managers, and Healthcare Navigators. Healthcare Navigators focus on

      • breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities,
      • introducing a better way to coordinate health care, and
      • providing high-need members with in-person care management where they live.
      Case Managers focus on providing services that help address members' health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care. These include

      • a variety of other community-based services.

The multi-story home sits on a large wooded lot, in a quiet and serene setting. Freedom House is located far away from the hustle and bustle of Santa Cruz, with convenient public transportation access. At Freedom house, clients receive the benefits of community support from recovery peers as well as bilingual house management. Residents build community connections by attending weekly house meetings where rules and policies are frequently reviewed and assessed which helps to ensure a safe and supportive environment for all residents.
