ENCOMPASS RETURN PROJECT - ADULT JUSTICE INVOLVED THERAPY AND CASE MANAGEMENT SERVICES
The ReTURN program provides therapy and mental health services for AB109 eligible probation clients who are adjusting to life after serving a jail or prison term. ReTURN helps clients gain pro-social skills, repair relationships, and attain and maintain sobriety. Our mission is to provide comprehensive bio-psycho-social assessments, individual and family cognitive-behavioral psychotherapy, and system navigation management to assist clients in transitioning from incarceration back into the community. FOSTER, PROBATION, OR HOMELESS TEEN/YOUNG ADULT (15-24) TRANSITIONAL HOUSING SERVICES - ENCOMPASS TAY PROGRAM
Transitional Housing Plus is a supportive housing program that serves former foster and probation youth between the ages of 18-24. Participants receive assistance securing independent housing in the community and meet regularly with their THP Coordinators to work on independent living goals. Throughout this process, participants receive financial assistance with rent, utilities, food, and educational expenses while also saving money for when they leave the program. They also receive emotional support, life skills coaching, and connections to valuable community resources.
DAMIANS LADDER
Damians Ladder assists low- and moderate-income older and disabled homeowners by providing minor home repairs that they cannot perform due to lack of funds and/or inability to physically perform the repairs themselves. Our purpose is to enable people to remain in their homes as long as possible.
FAMILY HEALTH PROGRAMS- HOME VISITING PROGRAMS
- Public Health Field Nursing A voluntary home visiting program with a Public Health Nurse to support families during pregnancy and with children up to 5 years old. The program provides strength-based case management, assists families with accessing resources, and provides health and parenting education.
- Nurse-Family Partnership (NFP) An evidence-based home visiting program that pairs first time expectant parents with a specially trained nurse. The program begins during pregnancy and provides support, education, and resources to families until their child turns 2 years old. Nurses also check in with the parent regarding their health and wellness and their child's growth and development.
- Comprehensive Perinatal Services Program (CPSP)CPSP is a statewide program that provides enhanced reimbursement for a wide range of services to Medi-Cal eligible pregnant and postpartum women.
- Sudden Infant Death Syndrome (SIDS) ProgramPublic Health Nurses in Santa Cruz County provide bereavement support, information, resources, and follow-up services to assist families and caregivers who have experienced the loss of an infant from SIDS or Sudden Unexpected Infant Death (SUID).
