CARe Team provides Case Management and treatments for individuals who would like to reduce their risk for becoming infected with HIV and those who are already infected.

Offers free supplies and services to injection drug users, including

• exchange of syringes
• HIV/HCV testing and prevention education
• referrals and linkages to treatment
• resources for primary care
• mental health and substance abuse services
• sharps containers and education on proper disposal of syringes
• 24/7 kiosks for sharp disposals
• Naloxone distribution
• harm reduction and safe sex supplies

Provides a wide range of services to homeless individuals in Santa Cruz County. Services provided include: information & referrals to other vital services such as

• benefits advocacy
• Medical and MediCare application assistance
• alcohol & drug counseling and support
• health education
• HIV testing
• hygiene supplies
• pet food
• case management.

All services are free and confidential. Also offers medical care and treatment at the following clinics. Most of the clinics are by appointment.
