CHDP program provided care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments were provided by enrolled private physicians, local health departments, community clinics, managed care plans, and some local school districts.
The CHDP program helps to prevent or find health problems through regular, no cost,
health check-ups. A check-up includes:
Health and developmental history
Physical exam
Needed shots
Oral health screening and routine
Referral to a dentist starting by age 1
Nutrition screening
Behavioral screening
Vision screening
Hearing screening

Lab tests, which may include: anemia, lead, tuberculosis, and other problems, as needed

Supportive relationships with patients, caregivers, and family members, Early childhood development support services, Referrals, Referral to other government and social service programs (e.g., for financial, food security, housing, etc.), Create opportunities for mentoring with caring adults (teachers, coaches, mentors), Early intervention services and supports, Parent education regarding child development, Early childhood development enrichment, Early childhood development including preschool enrichment.
