Provides early detection and prevention of health problems among children from low-to-moderate income families. Also offers a full range of health assessment services including:
  • health and developmental history
  • physical examination
  • nutritional assessment
  • immunizations
  • vision testing
  • hearing testing
  • lead testing
  • some laboratory tests
  • health education
  • anticipatory guidance, such as age appropriate safety awareness.

CHILDREN'S MEDICAL SERVICES

The Child Health and Disability Prevention (CHDP) program was a preventive program that delivered periodic health assessments and services to low-income children and youth in California. CHDP program provided care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments were provided by enrolled private physicians, local health departments, community clinics, managed care plans, and some local school districts.

The CHDP program helps to prevent or find health problems through regular, no cost,
health check-ups. A check-up includes:

Health and developmental history
Physical exam
Needed shots
Oral health screening and routine
Referral to a dentist starting by age 1
Nutrition screening
Behavioral screening
Vision screening
Hearing screening
Health information
Lab tests, which may include: anemia, lead, tuberculosis, and other problems, as needed
Referral to Women, Infants, and Children (WIC)
