RETURN PROJECT (RE-ENTRY SERVICES)


The ReTURN program provides therapy and mental health services for AB109 eligible probation clients who are adjusting to life after serving a jail or prison term. ReTURN helps clients gain pro-social skills, repair relationships, and attain and maintain sobriety.

Our mission is to provide comprehensive bio-psycho-social assessments, individual and family cognitive-behavioral psychotherapy, and system navigation management to assist clients in transitioning from incarceration back into the community.

OUTREACH-BASED CASE MANAGEMENT | PEOPLE FIRST OF SANTA CRUZ COUNTY

OUTREACH-BASED CASE MANAGEMENT

The Outreach-Based Case Management Program helps those who are experiencing homelessness in Santa Cruz County, pursue permanent housing solutions, access to community health and financial resources, provide housing navigation and supportive services to help individuals and families lead more stable lives. The Outreach-Based Case Management team consists of an Program Manager, Case Managers, and Healthcare Navigators.

Healthcare Navigators focus on
- breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities,
- introducing a better way to coordinate health care, and
- providing high-need members with in-person care management where they live.

Case Managers focus on providing services that help address members' health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care. These include
- support to secure and maintain housing,
- access to resources to support short term recovery, and
- a variety of other community-based services.

HOSPICE TRANSITION & GRIEF PROGRAM

Provides hospice care to terminally ill patients at home or in a skilled nursing facility.

Grief Support program provides support and counseling to surviving family members and friends. Assists them through the painful period following the death of a loved one.

Transitions program provides case management and non-medical supportive services for patients diagnosed with a progressive illness or end-stage disease who are not ready for hospice care.

Supportive relationships with patients, caregivers, and family members, Supportive relationships with patients and families, Supportive relationships with patients and families that are culturally and linguistically appropriate, Family support services, Coordinate referrals, data, and care, Referral to other government and social service programs (e.g., for financial, food security, housing, etc.), pediatric hospice with an interdisciplinary team - RN, Social Work, Spiritual Care, Music Therapy.

DISABILITY SERVICES | SAN ANDREAS REGIONAL CENTER

DISABILITY SERVICES

Provides a variety of diagnostic and prevention services to help those affected by severe autism, mental retardation, cerebral palsy, uncontrolled seizures and other developmental disabilities. Services include;
  • Case management and service coordination including planning, goal setting, residential placement, respite help, medical services, therapies, parenting classes, independent living skills training, mobility training and more.
  • Early intervention services for young children (age 0-3) with developmental disabilities may include speech therapy, occupational therapy, physical therapy, infant development programs, behavior modification and respite care.
  • School age services are provided for children and youth (age 3- 22). Offers assistance accessing generic resources, advocacy at Individual Education Plan (IEP) meetings, respite and camp, day care, residential placement, behavior modification and adaptive skills training.

Adults' services revolve around community inclusion, self-determination, and independence. Services deemed appropriate to meet an individual's needs may include
  • Assistance accessing community resources or entitlements such as SSI, day programs, supported employment, independent living skills training, supported living services, residential care home placement, respite, behavior modification, adaptive skills training and competency training (for court-related issues).

ADOLESCENT FAMILY LIFE PROGRAM

Provides home visits to pregnant and parenting teenagers, male (age 19 and under) and female (age 18 and under) and their children. Services include assessments, home nursing, parenting education, and case management to promote a healthy pregnancy and baby, to stay in school or re-enroll in school, to postpone future pregnancies, and to become emotionally, socially and financially independent.

FAMILIES TOGETHER | ENCOMPASS COMMUNITY SERVICES

FAMILIES TOGETHER

Families Together provides counseling, parenting support, and case management to families using a home visiting model and a client-centered approach to services. Caregivers participate in the development of an individualized service plan based on their unique strengths and needs.

Families referred by Santa Cruz County Family & Children's Services or CalWORKs are assigned a therapist who helps design customized services, including sessions in English or Spanish, and in the home or other preferred location. Families learn about parenting, child development, parent-child attachment, and general wellness. In addition to counseling, home visitors connect families with other community resources such as substance use counseling, interpersonal violence services, and early education support.

HOUSING NAVIGATION & SUSTAINING SERVICES

The Encompass Housing Navigation Team provides support to tenants of Encompass Housing in order to facilitate a stable living environment, help assist tenants in developing a support system, improve skills needed to maintain housing, and to connect with community resources for employment, education, medical needs and community involvement.

Services Offered
- Help finding and retaining permanent housing for individuals or families
- Help with home loans and financing
- Linkage to other community support services (dental, transportation, medical care, etc.) as needed
