Utility Bill Assistance:

We can assist eligible clients with utility bills (electricity, gas, and, in some cases, water) as part of housing stabilization and transition services.

Provides home-visiting counseling, parenting education, and case management to assist families with developing individualized service plans. Delivers support regarding child development, parent-child attachment, and general wellness. Connects participants to community resources, including substance use counseling, interpersonal violence services, and early education support.

The Outreach-Based Case Management Program helps those who are experiencing homelessness in Santa Cruz County, pursue permanent housing solutions, access to community health and financial resources, provide housing navigation and supportive services to help individuals and families lead more stable lives. The Outreach-Based Case Management team consists of an Program Manager, Case Managers, and Healthcare Navigators. Healthcare Navigators focus on

• breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities,
• introducing a better way to coordinate health care, and
• providing high-need members with in-person care management where they live.

Provides case management, service coordination, diagnostic services, and prevention support for individuals with developmental disabilities. Offers early intervention, educational advocacy, therapy, behavior modification, and adaptive skills training. Assists with independent living skills, respite care, residential placement, and community integration across all age groups.
