Description: Located just outside of Downtown Santa Cruz, CA, this 8-bed, all male facility provides a healthy balance of independence and support. The location provides easy access to public transportation and is in the heart of West Side Santa Cruz, which is bustling with activities and local businesses. At Weeks SLE, residents are encouraged to practice healthy relationships, accountability and responsibility. Active participation in some form of recovery program is required for residents, as well as attending regular house meetings and community chores. CLEVELAND HOUSE
Description: Cleveland House is a beautiful 6-bed, female-only facility which provides a safe environment for likeminded sisters in recovery. Located near busy Mission Street in Santa Cruz's beautiful Residential West Side, residents will have access to many local eateries, businesses, activities and job opportunities. Residents are encouraged to live independently in this supportive, community based setting. Residents are encouraged to develop and practice life skills and relationship building. In this supported environment, residents are able to develop a stronger sense of accountability, responsibility and community to further their recovery. FREEDOM HOUSEDescription: Located just off historic Highway 1 in Aptos, CA, Freedom House is a sober living home for 12 men who are actively engaged in recovery. The multi-story home sits on a large wooded lot, in a quiet and serene setting. Freedom House is located far away from the hustle and bustle of Santa Cruz, with convenient public transportation access. At Freedom house, clients receive the benefits of community support from recovery peers as well as bilingual house management. Residents build community connections by attending weekly house meetings where rules and policies are frequently reviewed and assessed which helps to ensure a safe and supportive environment for all residents. TAY (TRANSITION AGED YOUTH) TRANSITIONAL HOUSING PLUS

Transitional Housing Plus is a supportive housing program that serves former foster and probation youth between the ages of 18-24. Participants receive assistance securing independent housing in the community and meet regularly with their THP Coordinators to work on independent living goals. Throughout this process, participants receive financial assistance with rent, utilities, food, and educational expenses while also saving money for when they leave the program. They also receive emotional support, life skills coaching, and connections to valuable community resources.

CHILD AND ADOLESCENT BEHAVIORAL HEALTH SERVICES

Provides comprehensive, strengths-based, culturally and linguistically appropriate behavioral health services for Medi-Cal eligible children and youth who have moderate to severe behavioral health needs.

Services include assessment, treatment planning, psychotherapy (individual, family), case management, and psychiatric care (evaluation and medication management). Services delivered at County behavioral health clinic locations, community, or home-based settings, as clinically appropriate.

The Access Team provides 24-hour confidential crisis intervention and referral services (800-952-2335).

Provides substance use disorder prevention, early intervention, and treatment services across the continuum of care.

SANTA CRUZ RESIDENTIAL RECOVERY

Santa Cruz Residential Recovery offers a 30-bed co-ed residential program, serving clients for 30-90 days, based on individual needs. Staffed by a highly skilled and trained team, certified clinicians help clients focusing on issues surrounding recovery from substance use and co-occurring disorders. Program activities include evidence-based trauma informed and cognitive behavior therapies (CBT), Dialectical Behavioral Therapy (DBT), as well as focused individual and group counseling, pro-social life skill building, mindfulness and daily house meetings.
