• Medically Tailored Meals - Provides no cost home delivered fresh food boxes to help support and manage the member's chronic health conditions.

Provides the following CalAIM Community Support Services:

Housing Deposits: -Provides assistance with housing security deposits, utilities assistance. Members can also receive funding for medically necessary items like air conditioners, heaters, and hospital beds to ensure their new home is safe for move-in.

Housing Tenancy and Sustaining Services: -Provides support to maintain safe and stable tenancy once housing is secured, such as coordination with landlords to address issues, assistance with the annual recertification process, and linkage to community resources to prevent eviction.

Housing Transition Navigation: -Provides help to find, apply for, and secure housing to members experiencing homelessness or at risk of experiencing homelessness.

Recuperative Care (Medical Respite): -Provides short-term residential care for members with unstable housing who no longer require hospitalization, but still need to heal from an injury or illness. The residential care includes housing, meals, ongoing monitoring of the member’s condition, and other services like coordination of transportation to appointments.

Respite Services: -Provides short-term relief for member caregivers. Members may receive caregiver services in their home or in an approved facility on an hourly, daily, or nightly basis as needed.

Short-Term Post Hospitalization Housing: -Provides short-term housing (up to six months) for members who do not have a residence, and who have high medical or mental health and substance use disorder needs so they can continue recovery. To receive this support, members must also have been discharged from an inpatient clinical setting, residential substance use disorder treatment or recovery facility, residential mental health treatment facility, correctional facility, nursing facility, or recuperative care.

Provides comprehensive care management from a single lead care manager. Coordinates all health-related care. Includes coordination for physical health, mental health, dental care, and social services.

Serves the Following Adult Populations of Focus: -At Risk for Avoidable Hospitalization or ED use -Birth Equity -Experiencing Homelessness -Living in the Community and at Risk for LTC Institutionalization -Nursing Facility Residents Transitioning to the Community -Serious Mental Health and or SUD Needs

Serves the following Youth Population of Focus: -At Risk for Avoidable Hospitalization or ED use -Experiencing Homelessness-Homeless Families Or Unaccompanied Children and Youth -Serious Mental Health and or SUD Needs (includes children with high ACEs scores)
