RESIDENTIAL AND WITHDRAWAL MANAGEMENT

Provides a sub-acute withdrawal management program that offers closely supervised, 24-hour residential care in a comfortable, safe environment. Withdrawal management is offered as a stand-alone program, but most people transition to another of Janus' Recovery Programs when they are finished with the process.

DISABILITY SERVICES | SAN ANDREAS REGIONAL CENTER

DISABILITY SERVICES

Provides a variety of diagnostic and prevention services to help those affected by severe autism, mental retardation, cerebral palsy, uncontrolled seizures and other developmental disabilities. Services include;
  • Case management and service coordination including planning, goal setting, residential placement, respite help, medical services, therapies, parenting classes, independent living skills training, mobility training and more.
  • Early intervention services for young children (age 0-3) with developmental disabilities may include speech therapy, occupational therapy, physical therapy, infant development programs, behavior modification and respite care.
  • School age services are provided for children and youth (age 3- 22). Offers assistance accessing generic resources, advocacy at Individual Education Plan (IEP) meetings, respite and camp, day care, residential placement, behavior modification and adaptive skills training.

Adults' services revolve around community inclusion, self-determination, and independence. Services deemed appropriate to meet an individual's needs may include
  • Assistance accessing community resources or entitlements such as SSI, day programs, supported employment, independent living skills training, supported living services, residential care home placement, respite, behavior modification, adaptive skills training and competency training (for court-related issues).

TRANSITION AGE YOUTH | COUNTY OF SANTA CRUZ HUMAN SERVICES DEPARTMENT'S FAMILY & CHILDREN'S SERVICES

TRANSITION AGE YOUTH

The Family & Children's Services Transition Age Youth (TAY) program serves current and former foster youth age 15-24 with a range of services to assist youth in making a healthy transition to adulthood. Programs include counseling, skill-building, a drop-in center and subsidized supportive housing.

TAY programs include:
. The Independent Living Program (ILP) serves current and former foster and probation placement youth age 15-21. It assists youth to develop independent living skills to successfully transition to self-sufficiency and includes services such as one-on-one counseling, assistance in obtaining ID cards, driving permits and licenses and workshops on topics that assist youth in achieving personal, education and vocational goals.
. The Transitional Housing Program is a supportive housing program for former non-minor dependents and foster youth between the ages 18-24. Youth are supported with financial assistance, educational expenses, emotional support, life skills coaching, and referral to community resources to help them achieve the financial stability necessary to succeed in independent living.

NEW OPTIONS

Provides coordination of care and services to adults with traumatic brain injuries after the individual has completed traditional medical rehabilitation.

Offers three monthly support groups, quarterly education events and ongoing networking meetings with other agencies providing services to people who care about brain injury.

EL DORADO CENTER

Treatment Focus: Helping individuals step down from locked care
El Dorado Center (EDC) is a community-based, short-term treatment program for individuals who may be stepping down from locked care. An intensive, structured residential program, EDC is an unlocked, home-like environment facilitating the healing process in preparation for transitioning back to community living. Our staff provide individual and group counseling, crisis intervention, structured activities, community outings, and assistance with independent living skills and connecting to the community. A registered nurse is on staff and a County psychiatrist is available for medication monitoring and management.

Provides individualized substance abuse recovery services including detoxification, residential treatment, day treatment, transitional housing, medically assisted treatment (methadone), and DUI programs for both first-time and repeat offenders. Offers special programs for pregnant women and women with young children.

MEDI-CRUZ

Provides health care coverage for low income individuals who don't have private health insurance, Medicare or Medi-Cal. Services are covered for a limited period specific to the treatment of the qualifying condition. Clients are expected to renew their application every two to three months depending on condition.

IN HOME SUPPORTIVE SERVICES PUBLIC AUTHORITY

The Public Authority is responsible for enrolling caregivers into the In-Home Supportive Services Program, provides ongoing training to IHSS caregivers, provide free supplies to IHSS caregivers and manage a registry of IHSS caregivers. The Public Authority assists IHSS recipients that need a caregiver with referrals of potential caregivers.
