Second Story is a six-bed house that serves as a respite and a voluntary opportunity for individuals to learn to use relationships to move out of old roles and patterns. Staffed by "peers," (people with lived experience of mood swings, consuming fears, voices, visions . . .) who have learned to be with some discomfort. The household provides an opportunity to experience what change feels like, and to learn new responses through relationships with each other. With the expectation that everyone is capable and inherently knows what they need, both paid staff and guests create a space for those qualities to surface. Along with creating a home, we share stories to connect and understand each other's experiences, gently challenging ourselves to listen to the untold story. The hope is not to rebuild a culture of illness, but instead generate some sparkling moments of connection and trust.

Families Together provides counseling, parenting support, and case management to families using a home visiting model and a client-centered approach to services. Caregivers participate in the development of an individualized service plan based on their unique strengths and needs. Families referred by Santa Cruz County Family & Children's Services or CalWORKs are assigned a therapist who helps design customized services, including sessions in English or Spanish, and in the home or other preferred location. Families learn about parenting, child development, parent-child attachment, and general wellness. In addition to counseling, home visitors connect families with other community resources such as substance use counseling, interpersonal violence services, and early education support. The multi-story home sits on a large wooded lot, in a quiet and serene setting. Freedom House is located far away from the hustle and bustle of Santa Cruz, with convenient public transportation access. At Freedom house, clients receive the benefits of community support from recovery peers as well as bilingual house management. Residents build community connections by attending weekly house meetings where rules and policies are frequently reviewed and assessed which helps to ensure a safe and supportive environment for all residents. MEDICATIONS FOR ADDICTION TREATMENT SERVICES - ENCOMPASS MAT PROGRAM

Our low-barrier outpatient MAT program provides rapid support to people struggling with substance use disorders, offering medication management as well as peer support. Our team includes two advanced practice providers, a nurse and a peer specialist to assist clients with their needs. We can see most clients within 24 hours and can do so in-person or virtually to avoid long wait times for appointments. Our peer specialist works with our community partners including the courts, emergency departments, residential facilities and outreach programs to support clients transitioning into our outpatient care. We also offer a contingency management program for clients struggling with either stimulant use disorder alone, or in addition to other substance use disorders. We are able to provide care for underinsured and uninsured individuals.

It is the goal of the Seniors Council Falls Prevention Program to reduce the number of debilitating falls suffered by older adults and persons with disabilities in Santa Cruz and San Benito counties by offering services to help older adults maintain healthy activity levels and providing the tools they need to avoid placing themselves at unnecessary risk. These services consist of: A Matter of Balance: an eight-session workshop for individuals who live with a fear of falling. Companion for Life: providing a low-cost emergency response system for at-risk adults in Santa Cruz and San Benito County.
