OUTREACH-BASED CASE MANAGEMENT

The Outreach-Based Case Management Program helps those who are experiencing homelessness in Santa Cruz County, pursue permanent housing solutions, access to community health and financial resources, provide housing navigation and supportive services to help individuals and families lead more stable lives. The Outreach-Based Case Management team consists of an Program Manager, Case Managers, and Healthcare Navigators. Healthcare Navigators focus on

  • breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities,
  • introducing a better way to coordinate health care, and
  • providing high-need members with in-person care management where they live.
Case Managers focus on providing services that help address members' health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care. These include

  • a variety of other community-based services.

Pregnancy Resource Center of Santa Cruz offers no-cost and low-cost services to women in need of pregnancy tests, ultrasounds, prenatal care, well-woman care, std testing & treatment and counseling services. Treatment Focus: Helping individuals maintain sobriety and transition to independent living Casa Pacific is part of an innovative project centered around work and meaningful daily activities, as the core of the co-occurring disorders treatment approach. Residents attend the Community Connection's Mariposa Center to learn vocational skills and participate in various volunteer jobs, or they participate in the Community Action Board's Alcanse work crew. Other activities include socialization, recreation, accessing community resources, on-site recovery meetings and individualized attention to assist residents in maintaining sobriety, reaching their personal goals, and transitioning into independent community living

It is the goal of the Seniors Council Falls Prevention Program to reduce the number of debilitating falls suffered by older adults and persons with disabilities in Santa Cruz and San Benito counties by offering services to help older adults maintain healthy activity levels and providing the tools they need to avoid placing themselves at unnecessary risk. These services consist of: A Matter of Balance: an eight-session workshop for individuals who live with a fear of falling. Companion for Life: providing a low-cost emergency response system for at-risk adults in Santa Cruz and San Benito County.
