Our Dientes Cares Program focuses on making oral health care available to all, especially for low-income, uninsured patients. For those patients who do not have dental insurance, Dientes offers a sliding scale fee (based on proof of income).

The Delta Dental Community Care Foundation, the philanthropic arm of Delta Dental of California, awarded Dientes Community Dental Care and Salud Para La Gente membership as the Senior Oral Health Coalition Program. The multi-year program launched in April 2022 to address the oral health crisis among older adults in the U.S. Our coalition promotes the connection between oral health and overall health, improve access and create greater equity in the care and treatment older adults receive.

Sueños provides job readiness, career development, paid work experience/internships, academic support, and other supportive services to eligible youth in Santa Cruz County. Youth Services: ACADEMIC SUPPORT, CAREER EXPLORATION AND DEVELOPMENT, SOCIAL-EMOTIONAL SUPPORT

Provides outreach, education, screening, referral, and enrollment assistance to community members seeking Medi-Cal coverage and re-enrollment support. Services include helping individuals understand eligibility requirements, complete renewal applications, submit required documents, update case information, and connect with county or community resources. Assistance is provided through community outreach events, phone support, appointments, and referrals to partner organizations to help ensure continued access to healthcare coverage. Additional support for labor rights education and referrals.

The Outreach-Based Case Management Program helps those who are experiencing homelessness in Santa Cruz County, pursue permanent housing solutions, access to community health and financial resources, provide housing navigation and supportive services to help individuals and families lead more stable lives. The Outreach-Based Case Management team consists of an Program Manager, Case Managers, and Healthcare Navigators. Healthcare Navigators focus on

• breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities,
• introducing a better way to coordinate health care, and
• providing high-need members with in-person care management where they live.

The ReTURN program provides therapy and mental health services for AB109 eligible probation clients who are adjusting to life after serving a jail or prison term. ReTURN helps clients gain pro-social skills, repair relationships, and attain and maintain sobriety. Our mission is to provide comprehensive bio-psycho-social assessments, individual and family cognitive-behavioral psychotherapy, and system navigation management to assist clients in transitioning from incarceration back into the community. Pregnancy Resource Center of Santa Cruz offers no-cost and low-cost services to women in need of pregnancy tests, ultrasounds, prenatal care, well-woman care, std testing & treatment and counseling services. Material assistance such as diapers, baby clothes and baby furniture are available to clients. We accept Medi-Cal. Spanish speaking counseling is available on certain days.

Encompass' Youth Services Mental Health Counseling program is personalized to meet the individual needs of youth and to enhance the strengths and assets of each family. We provide individual, group, and family counseling for youth and families. Participating in counseling improves family, peer, and community relationships; increases self-confidence and school performance; and reduces drug and alcohol use. Services can be provided in home, in office, virtually via the web, and over the phone.
