EATING DISORDERS PARTIAL HOSPITALIZATION PROGRAM (PHP)

The Lotus Collaborative Partial Hospitalization Program (PHP) for eating disorder recovery offers clients 5-6 hours of support, 5-7 days a week. Treatment days of our PHP include group therapy, nutrition education, a weekly nutrition experiential, two supervised meals and one supervised snack. Clients receive two weekly individual therapy sessions, weekly individual dietary sessions with a registered dietitian as well as a weekly psychiatry session as clinically indicated.

MEDI-CAL MANAGED HEALTH CARE PLAN

Provides needed health care services for low-income individuals including families with children, seniors, persons with disabilities, foster care, pregnant women, and low-income people with diseases such as asthma, diabetes, tuberculosis, breast cancer, or HIV/AIDS. Eligibility is determined by the Santa Cruz County Health Services department.

Transportation Services

If you are medically unable to use a car, bus, train or taxi, the Alliance will arrange transportation for you. This type of transportation is called non-emergency medical transportation (NEMT).

NEMT is for when you need extra help or special transportation needs. Examples of NEMT are an ambulance, wheelchair van or air transport.

Transportation to medical services by passenger car, taxicab or other forms of public or private transportation is called non-medical transportation (NMT). NMT is available to members who need help getting to medical services. Members must show that they do not have any other transportation options.

OUTREACH PROGRAM

Our Outreach Program eliminates transportation as a barrier to care for underserved Santa Cruz County residents at many venues, including schools, skilled nursing facilities (SNFs), and our one-chair Outreach Clinic at Housing Matters in Santa Cruz. Our partnerships provide patients who might not otherwise get access to dental services to the treatment they need. At our outreach sites, our staff delivers oral health evaluations including x-rays, and procedures such as cleanings and fluoride varnish. Those patients who require more complex care are referred back to our clinic. Dientes provides oral health education and preventive services in six school districts (Pajaro Valley Unified, Live Oak, San Lorenzo Valley, Soquel Unified, Santa Cruz City & Santa Cruz County Office of Education) at 30+ schools. The program has been a huge success since its inception in 2009.

PUBLIC GUARDIAN

The County Of Santa Cruz Human Services Department's Public Guardian manages the property, finances and personal care needs of county individuals who are substantially unable to provide for themselves. This is accomplished through conservatorships. If you are concerned about the well-being of an individual who is elderly and unable to provide for or manage their personal needs, or who is gravely disabled as a result of a mental disorder and unable to provide for their basic necessities, you can make a referral to the Public Guardian to investigate the need for a conservator.

Works with local schools to promote bicycle safety through outreach events and hands-on training sessions. The goal of the program is to reduce the collision rates of young bicyclists, increase safety, promote legal bicycling practices among youth, and increase use and proper fit of bicycle helmets.
